Global Impact of 2017 American Heart Association/American College of Cardiology Hypertension Guidelines: A Perspective From Japan
Abstract
he 2017 American Heart Association/American College of Cardiology (AHA/ ACC) guidelines for the prevention, detection, evaluation, and management of high blood pressure (BP) in adults were recently released.1 The core concept in these guidelines is earlier and tighter BP control over 24 hours, with the aim of providing more sustained target organ protection and cardiovascular disease prevention. These guidelines lower the threshold of hypertension diagnosis and target BP levels to 130/80 mm Hg.This direction of strict BP control with a new definition and universal target goal for hypertension will be the topic of debate among the guideline developing committee members of the Japanese Society of Hypertension. 2

In the 2008 ESH HBPM recommendations, a threshold for hypertension diagnosis with systolic/diastolic HBP of 135/85 mmHg was proposed, based on a review of evidence including two meta-analyses.
“In the 2008 ESH HBPM recommendations, a threshold for hypertension diagnosis with systolic/diastolic HBP of 135/85 mmHg was proposed [4], based on the review of evidence including two meta-analyses [139,140]”
Treated hypertensive patients have higher day-by-day BPV compared with untreated individuals.
“Studies focusing on treated hypertensive patients have found a higher day-by-day BPV among these individuals compared with untreated individuals [38,140], also reporting higher values of HBPV in case of treatment with beta-blockers, short duration of treatment [246] and increasing number of antihypertensive drugs [247].”

About these checks
Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.
Statistics and arithmetic are recomputed, not judged.
Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
