Cervical Cancer Elimination in India: Progress, Gaps, and the Priorities
Open accessAbstract
Background: Cervical cancer is the second most common cancer and cause of cancer death among women in India, with an estimated 79,000 new cases and 43,000 deaths in 2024. Following the February 2026 nationwide HPV vaccination rollout, this review assesses India’s progress toward the WHO 90– 70– 90 elimination targets and identifies the gaps most likely to determine further progress. Methods: We conducted a critical narrative review informed by the authors’ program experience and targeted searches from publicly available databases, including national program documents, government reports, national surveys and WHO guidelines until September 2026. We selected evidence relevant to vaccination, screening, and treatment in India and synthesised it thematically. Results: The national program targets over 10 million girls aged 14 years with a single HPV vaccine dose. About 8 million were vaccinated till September 2026with very few serious adverse events reported. Facility-only delivery, mandatory written consent and limited supply for additional cohorts are major barriers to achieve the 90% target. Screening coverage remains low [< 2%], with continued reliance on VIA, no harmonised performance indicators and weak linkage to care. Randomised trials support primary HPV testing, and newly validated indigenous assays could make this transition affordable, although triage requirements and overtreatment need attention. Limited colposcopy, loss to follow-up and insufficient radiotherapy capacity constrain treatment. Conclusion: India has the health-system architecture, manufacturing capacity and digital tools needed for elimination, but remains substantially short of all three targets. Progress will depend mainly on: i) extending vaccination beyond facilities and to more cohorts ii) moving to organised, self-sampled HPV-based screening iii) closing the gap between screening and treatment. Keywords: HPV vaccine, early detection of cancer, India, uterine cervical neoplasms, human papillomavirus DNA tests

TL;DR
This critical narrative review assesses India’s progress toward WHO cervical cancer elimination targets after the February 2026 nationwide HPV vaccination rollout. It finds substantial health-system enablers but concludes India remains substantially short of all three 90-70-90 targets, with major gaps in vaccination reach, HPV-based screening, and linkage to treatment.
Key findings
- India launched nationwide HPV vaccination in February 2026 for a single cohort of 14-year-old girls, targeting more than 10 million beneficiaries; about 8 million were vaccinated by September 2026, with very few serious adverse events reported. Facility-only delivery, mandatory written consent, and limited supply for additional cohorts are major barriers to the 90% target.
- Screening coverage remains low: NFHS-5 reported 1.9% ever screened among women aged 30–49 in 2019–2021, while administrative records reported 87.3 million cumulative cervical screenings by February 2026, with 93,062 cervical cancer diagnoses and 78,676 women on treatment. The program still relies on VIA, lacks harmonized performance indicators, and has weak linkage to care.
- Treatment capacity is constrained: India had 823 functioning radiotherapy machines across 554 facilities, a national teletherapy density of 0.6 machines per million population, and a shortfall of 1,209 machines. Population-based survival data show five-year cervical cancer survival ranging from approximately 32% to over 60%, with poorer outcomes in rural and resource-constrained regions.
- An indigenous HPV test validated in 2025 showed sensitivity of 80.4% (95% CI 71.1–87.8) and specificity of 91.5% (95% CI 89.5–93.2) for detection of underlying CIN2+, supporting a possible transition to HPV-based screening if triage and treatment systems are ready.
Methods
- Critical narrative review informed by the authors’ program experience and targeted searches of publicly available databases, national program documents, government reports, national surveys, and WHO guidelines until September 2026.
- Evidence was drawn from government documents, public data sources such as NFHS and NCRP, WHO/IARC/national technical body guidance, and peer-reviewed publications identified through Google Scholar; reference lists of key publications were also screened.
- Evidence was synthesized thematically under the three WHO elimination pillars—HPV vaccination, screening, and treatment—and India’s status was compared against the WHO 90-70-90 interim targets. No ethical approval was required.
Limitations
- The paper states it is a narrative review rather than a systematic one; relevant evidence may have been missed, and source selection may reflect the authors’ judgement. Much information on national programs comes from government documents, administrative reports, and press releases that vary in quality and have not been independently verified.
- The paper states that HPV vaccination information is from the initial stages of national rollout and could not be used for broader interpretations; the reported ~8 million vaccinated against an initial eligible cohort of ~11.5 million girls aged 14 gives an approximate ratio of 43%, described as an early administrative indicator, not verified coverage by age 15.
- The text makes evident that administrative screening and treatment indicators do not establish high-performance screening coverage at both target ages or treatment completion/appropriateness: test modality, unique participants, screening interval, eligible denominators, and separate WHO-compatible precancer and invasive cancer coverage estimates require clarification.

InterpretationcitationAlthough triage-based strategies may reduce overtreatment, simplified single-visit approaches remain critical in settings where follow-up is uncertain, as additional diagnostic steps may inadvertently exclude women who would otherwise benefit from timely treatment.
“Taken together, these findings suggest that although triage-based strategies may reduce overtreatment, simplified single-visit approaches remain critical in settings where follow-up is uncertain, as additional diagnostic steps may inadvertently exclude women who would otherwise benefit from timely treatment.44”

InterpretationreasoningProgress toward cervical cancer elimination in India will depend mainly on extending vaccination beyond facilities and to more cohorts, moving to organised self-sampled HPV-based screening, and closing the gap between screening and treatment.

InterpretationreasoningThese disparities are closely associated with differences in availability of diagnostic and treatment services, workforce distribution, and continuity of care, with patients in lower resource settings more likely to experience delayed diagnosis, incomplete treatment, loss to follow-up, perceived stigma, and catastrophic health expenditure.

About these checks
Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.
Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-10-16.
Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-10-16.
Statistics and arithmetic are recomputed, not judged.
Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
