Treatment of the autonomous thyroid nodule: a review

Ferrari, Carlo, Reschini, Eugenio, Paracchi, Alessandra·1996·European Journal of Endocrinology

Abstract

Pathophysiological and clinical aspects of the autonomous thyroid nodule have been reviewed previously (1, 2). Interest in this disease has been renewed in recent years by exciting molecular studies showing that somatic mutations activating the TSH receptor are the main cause of autonomous adenoma (3–6). For the purposes of the present discussion on the treatment of this disease, it is sufficient to recall that it may present at one of three clinical stages (1, 2, 7–9): "compensated" nodule, i.e. a nodule that is hot or warm at scintigraphy performed with a radioisotope of iodine (to exclude the possibility of a nodule of the trapping-only type (10)) but that fails to inhibit completely or substantially the radionuclide uptake by the extranodular tissue; patients with compensated nodules have detectable and usually normal serum TSH, as well as normal free (F) thyroid hormone levels (FT3 and FT4); non-toxic or "pretoxic" nodule, i.e. a hot nodule without consistent extranodular uptake, with undetectable TSH, but FT3 and FT4 still within the normal range; toxic nodule, i.e. a nodule that inhibits TSH secretion and extranodular uptake and also secretes thyroid hormones in sufficient amounts to induce supranormal FT3 and/or FT4 serum levels.

About these checks

Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.

Statistics and arithmetic are recomputed, not judged.

Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
