Abstract

Please cite this paper as: Vatish M, Steer P, Blanks A, Hon M, Thornton S. Diurnal variation is lost in preterm deliveries before 28 weeks of gestation. BJOG 2010; DOI: 10.1111/j.1471‐0528.2010.02526.x. Preterm delivery is the primary cause of neonatal mortality and morbidity worldwide. Labour at term occurs as a culmination of maturational events in both the fetus and maternal uterus. This process exhibits diurnal variation, with the onset of labour being more common at night. We have confirmed that this diurnal variation is present in gestations between 28 and 36 weeks, but is absent below 28 weeks. We hypothesise that this is because before 28 weeks of gestation, the onset of labour may result from a pathological rather than a physiological process. This may have important implications regarding any pharmacological approach to the prevention and/or treatment of very early preterm labour.

  • Prior studies on circadian patterns on the time of the onset of preterm labour provide conflicting results.

  • Both normal labour onset and term premature rupture of membranes are more common at night; some authors have suggested an altered diurnal circadian pattern for PTL or very preterm spontaneous onset of labour, but not for PPROM.

About these checks

Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.

Statistics and arithmetic are recomputed, not judged.

Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
