Incretin effect potentiates β-cell responsivity to glucose as well as to its rate of change: OGTT and matched intravenous study

Abstract

The aim of this study is to gain greater insight into the mechanism whereby “incretins” (greater insulinemia after oral than intravenous glucose) enhance insulin secretion. To do so, we use a model of C-peptide secretion to reanalyze data from a previously published study in which glycemic profiles observed following glucose ingestion were matched in the same 10 subjects by means of an intravenous glucose infusion. We report that incretins increase insulin secretion by enhancing both the dynamic (to the rate of increase of glucose) and static (to given glucose concentration) response with an increase of 58% for the static (Φ s = 16.4 ± 1.8 vs. 24.6 ± 2.0 10−9 min−1, P = 0.01) and 63% for the dynamic (Φ d = 278 ± 32 vs. 463 ± 86 10−9, P = 0.02) indexes. Since increases in the dynamic response to glucose are believed to be due to an increase in the rate of docking, and exocytosis of insulin containing granules and increases in the static response to glucose are believed to be caused by a shift in the sensitivity of the β-cell to glucose, these results suggest that incretins may modulate more than one step in the β-cell insulin secretory cascade.

Claims other papers make about this one

Each is checked against this paper’s own text: does it say what the citing paper says it does?

  • In Ref. 5, the difference between oral and intravenous β-cell responsivity indexes provided an indirect measure of the incretin effect; there was an approximately 60% difference between the two indexes, implying that the incretin effect is responsible for such potentiation.

“The difference between oral and intravenous indexes provided an indirect measure of the incretin effect; there was an ∼60% difference between the two indexes, implying that the incretin effect is responsible for such potentiation.”
  • The percentage increases of Φd and Φs due to incretins were similar (58 and 63%, respectively) in Ref. 5.

About these checks

Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.

Statistics and arithmetic are recomputed, not judged.

Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
