Deterministic Integrity Gates for LLM-Assisted Clinical Manuscript Preparation: An Auditable Biomedical Informatics Architecture
Open accessAbstract
As autonomous research agents and AI co-scientist systems push large language models (LLMs) from drafting toward end-to-end manuscript production, the bottleneck shifts from generation to verification. Fluent LLM output can hide fabricated citations, numbers that drift from source tables, and unmet reporting-guideline items; existing tools generate without verifying, and self-critique inherits the blind spots that produce confident fabrication. We describe an architecture pairing generation with verification, resting on three principles: decompose the workflow into self-contained skills, gate every stage transition with halt-on-failure, and resolve each integrity question with the cheapest sufficient mechanism, a deterministic, re-executable check where one suffices and a prose-level probe only where interpretation is unavoidable. This determinism-where-possible split, organized as an integrity-gate taxonomy, is the core contribution. It is realized as MedSci Skills, an open-source toolkit of 43 skills with a 21-detector deterministic tier, evaluated on three public-dataset pipelines (STARD, PRISMA, STROBE) and a seeded-defect ablation. Across the three pipelines every content-hash manifest verified clean and the gates surfaced real defects; on 27 identical injected defects the deterministic gates detected all 27 with no false positives on the matched clean fixtures, whereas a single-prompt LLM reviewer detected 11, its misses in code, bibliography, and style defects the prose hides. Determinism-where-possible verification yields an auditable, re-executable trail that exposes the evidence a human needs to check an LLM-assisted manuscript: feasibility and reproducibility evidence, not a claim of human-competitive quality, which a separate blinded study addresses. MedSci Skills is MIT-licensed and archived (v3.8.0).

TL;DR
The paper presents MedSci Skills, an open-source architecture of 43 skills and 21 deterministic standard-library integrity detectors that gates LLM-assisted clinical manuscript preparation with halt-on-failure checks. In three public-dataset demonstrations and a 27-defect seeded benchmark, deterministic gates detected 27/27 injected defects with 0 clean false positives, versus 11/27 for a generic single-prompt LLM reviewer.
Key findings
- Across three pipelines (STARD 2015, PRISMA 2020, STROBE), every content-hash manifest verified clean; gates surfaced real defects, including a prognostic claim in a cross-sectional study corrected to association-only language. Headline results included Wisconsin Breast Cancer logistic-regression AUC 0.998 (95% CI 0.994–1.000), BCG meta-analysis pooled RR 0.49 (95% CI 0.34–0.70; I² = 92.2%), and NHANES 2017–2018 adjusted OR 3.03 (95% CI 2.29–4.02).
- On 27 offline injected defects from 17 distinct injectors, deterministic gates recovered 27/27 (100%) with 0 clean false positives; a generic single-prompt LLM reviewer recovered 11/27 (41%), missing style/review-process, generated-code, and bibliography-internal classes the prose does not expose.
- Audit-trail probe extracted 249 manuscript claims, of which 136 were numerical; 74 (54%) exact-matched a manifest-locked analysis table value, 48 (35%) were partial matches, and 14 (10%) were untraced.
- Reproducibility/portability checks: 3/3 demonstration manifests verified clean on independent checkout; 21 deterministic detectors, 18 with a regression test and 17 with a synthetic fixture; 4/4 injected metadata drifts caught; installer self-test found all 43 skills discoverable, 0 personal-path leaks, and 4/4 documented host targets checked by installer self-test and path-contract scan (not live in-host execution).
Methods
- Design: manuscript preparation is decomposed into self-contained skills coordinated by an orchestrator, with a verification gate at every stage transition and halt-on-failure; the integrity-gate taxonomy resolves each question with the cheapest sufficient mechanism—deterministic re-executable checks where a lookup or arithmetic identity suffices, prose-level probes only where interpretation is unavoidable.
- Data and evaluation: MedSci Skills v3.8.0 (43 skills; 21 standard-library detectors in five families) was evaluated on three end-to-end demonstrations regenerated on v3.7.0 from public datasets—Wisconsin Breast Cancer (STARD 2015), BCG vaccine trials (PRISMA 2020), and NHANES 2017–2018 (STROBE)—plus a seeded-defect harness injecting one known defect at a time into temporary copies and an ablation substituting a generic single-prompt LLM review on the identical 27 defects.
- Analysis: deterministic code executed by the first author; content-hash manifests record SHA-256 over inputs and derived tables with per-column value hashes; audit-trail probe extracts claims and classifies exact/partial/untraced matches; portability checked by installer self-test and path-contract scan.
Limitations
- Stated: the toolkit was primarily authored by one of the authors and applied its own quality checks to this manuscript; human-judged labels were adjudicated by the developer-author; the skill set reflects one physician-researcher’s domains; the paper is a software-and-methods report not targeted by the toolkit’s clinical-manuscript machinery, producing two false positives; evidence is feasibility by demonstration with external independent use untested; the seeded-defect benchmark measures recall on known injected defects, not false-negative rate on real manuscripts or precision; two network-required citation injectors were not run; the ablation used one model, one fixed prompt, one date; no human-competitive quality claim is made.
- Stated: the demonstrations’ references were left as unverified placeholders by design; self-review scores are the toolkit’s internal grader output, not an external quality measure; reporting-compliance summaries are descriptive only (n = 3); portability was checked by installer self-test and path-contract scan rather than live in-host execution.
- Evident from text: the authors disclose a non-financial conflict of interest because Y.N. is founder of Aperivue, under which MedSci Skills is developed and released, and state they do not claim to be disinterested evaluators.

Recursive Self-Improvement in AI: From Bounded Self-Refinement to Autonomous Research Loops
Chen, M., Wang, L., & Qu, B. (2026) · arXiv
“…teachers that transmit in-domain bias with token-level efficiency [111]; AI scientists whose self-critique “inherits the blind spots that produce confident fabrication” [142]; and — at the integrity extreme — systems that misreport failure as success when task completion is rewarded [143].”

About these checks
Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.
Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-10-07.
Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-10-07.
Statistics and arithmetic are recomputed, not judged.
Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
