Glucocorticoid-induced osteoporosis

Open access

Abstract

Glucocorticoid treatment can alter bone metabolism, reduce bone strength, and increase the risk for osteoporotic fractures. Risk factors for glucocorticoid-induced osteoporosis (GIO) include older age, high doses, and longer duration of glucocorticoid use. The bone loss that accompanies glucocorticoid use is rapid, and early treatment with bone-sparing agents can prevent bone loss and reduce fracture risk. Several randomized controlled clinical trials have found prevention and treatment of GIO with bisphosphonates, and recently the treatment of GIO with teriparatide, to be effective. This article reviews current information on the epidemiology, pathophysiology, and clinical studies that support using bone-active agents to prevent and treat GIO.

Reid DM, Devogelaer J, Saag K,: Effect of a single infusion of zoledronic acid 5mg versus oral risedronate 5mg on bone mineral density at lumbar spine, hip, femoral neck and trochanter in patients with glucocorticoid induced osteoporosis [abstract F403]. J Bone Miner Res 2008, 23: s122. This is a randomized, controlled clinical trial of individuals on glucocorticoids and with osteopenia who were treated with intravenous zoledronic acid or oral risedronate. Zoledronic acid–treated patients had a greater increase in lumbar spine and hip BMD compared with the risedronate-treated patients. Therefore, in patients on glucocorticoids who could take an oral medication, zoledronic acid (administered intravenously) was rapid in onset and improved bone mass.

This abstract was presented in September 2008 at the American Society of Bone and Mineral Resarch. It reported on individuals on glucocorticoids who were randomly assigned to teriparatide or alendronate for 36 months. Those on teriparatide continued to have a reduced risk of incident vertebral fractures compared with individuals treated with alendronate for 36 months.

About these checks

Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.

Statistics and arithmetic are recomputed, not judged.

Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
