Construction of nursing-sensitive quality indicators for hemodialysis vascular access: a Delphi study
Open accessAbstract
Aims: The aim of this study was to develop a set of quality indicators for hemodialysis vascular access nursing. Design: A two-round Delphi study was conducted from August to September 2025. Methods: Based on the Donabedian structure–process–outcome theory model, this study retrieved, evaluated, and synthesized the best evidence on quality indicators for hemodialysis vascular access nursing. A questionnaire was developed through evidence-based analysis and semistructured interviews. An indicator system was established using the Delphi method, and an analytic hierarchy process (AHP) was employed to determine the weights of each indicator. A total of 16 experts from 14 hospitals across nine provinces in China completed two rounds of Delphi surveys. Results: After two rounds of consultation, the experts reached a consensus on the definitions of the indicators, calculation formulas, and data collection methods. They established a quality indicator system for hemodialysis vascular access nursing, which includes 3 primary indicators and 15 secondary indicators. Conclusion: The developed nursing quality-sensitive indicator system for hemodialysis vascular access was constructed through a rigorous Delphi process and aligns with the practical features of hemodialysis vascular access nursing management. It may serve as a reference basis for evaluating the quality of vascular access nursing, pending further clinical validation. Implications for the profession and patient care: This framework offers a structured approach to monitoring vascular access nursing quality. Its potential to reduce complications and improve patient outcomes requires confirmation through future clinical implementation studies. Impact: The indicators identified in this study are highly consistent with those in clinical practice, demonstrating sensitivity and practicality. The hemodialysis centers of relevant medical institutions may consider using this quality indicator system for routine data collection. Its utility for quality improvement should be further evaluated in clinical practice. Reporting method: This study was reported in line with the Conducting and Reporting of Delphi studies (CREDE) guidance on Delphi studies.

TL;DR
This two-round Delphi study developed nursing-sensitive quality indicators for hemodialysis vascular access using the Donabedian structure–process–outcome model and analytic hierarchy process. It produced a final system of 3 primary and 15 secondary indicators, pending further clinical validation.
Key findings
- After two rounds with 16 experts, consensus was reached on 3 primary indicators and 15 secondary indicators; questionnaire response rate was 100% in both rounds, expert authority coefficient Cr = 0.904, and Kendall’s W ranged from 0.136 to 0.310 (p < 0.05).
- AHP weights gave structural indicators the highest weight (0.6483), followed by outcome indicators (0.2297) and process indicators (0.1220); the top-ranked secondary indicator was nurse-to-patient ratio (0.3498), followed by establishment of a vascular access monitoring team (0.1925) and pass rate of vascular access nursing training and assessment (0.1059).
- The highest-weighted outcome indicator was incidence of central venous catheter-related infections (0.1049), followed by long-term utilization rate of arteriovenous fistulas (0.0447); the highest-weighted process indicators were correct execution of hand hygiene rate (0.0394) and correct execution rate of physical examination for arteriovenous fistula (0.0310).
- Indicator refinement moved from 3 primary/18 secondary indicators after literature review to 3/16 after interviews, then to 3/15 after two Delphi rounds; first-round removals included “ultrasound utilization rate for new fistula assessment” and “incidence of poor internal fistula function.”
Methods
- Two-round Delphi study conducted August–September 2025, based on Donabedian structure–process–outcome theory, with literature review (2,423 articles retrieved; 22 included), semistructured interviews with 6 experts, and questionnaire development.
- Sixteen experts from 14 hospitals across nine provinces/municipalities in China completed both rounds; mean age was 41.74 ± 5.86 years and mean work experience was 21.38 ± 7.50 years.
- Indicator screening used mean importance score >3.5, coefficient of variation <0.25, and full-score rate >20%; AHP used Saaty’s 1–9 scale with consistency ratio <0.1 considered acceptable; analyses used Excel 2016, SPSS 26.0, and R 4.5.2.
Limitations
- Stated: the indicator system has only been validated by expert consensus and has not been tested in real clinical settings; practical operability, data accessibility, inter-rater reliability, and associations with clinical outcomes remain to be verified.
- Stated: all experts were recruited from within China, which may introduce regional and cultural biases; the panel excluded patients and caregivers, and application outside China will require adaptive adjustment and local validation.
- Evident from the text: no predefined thresholds were set for outcome indicators, and process indicators such as hand hygiene and physical assessment may be difficult to collect continuously because they require real-time observation and additional manpower.

The Discussion reports consensus on 26 haemodialysis nurse-sensitive indicators. Puncture-injury incidence uses different denominators
MajorThe definition divides puncture injuries by the number of hemodialysis cases using fistulas, but the calculation formula divides by the number of patients with fistulas. Patients may have multiple hemodialysis sessions, so these denominators produce different incidence values.
“The ratio of the number of cases of arteriovenous fistula puncture injuries to the total number of hemodialysis cases via arteriovenous fistulas during the statistical period, including both autologous and graft arteriovenous fistulas.”
“Number of puncture injuries in arteriovenous fistulas/Total number of hemodialysis patients with arteriovenous fistulas during the statistical period * 1000‰”
Physical-examination rate counts patients in its definition but examinations in its formula
MinorThe definition uses the number of patients examined as its numerator, while the formula uses the number of examinations performed. Repeated examinations of one patient would give different rates.
“The ratio of the number of patients who underwent correct physical examinations by nurses before arteriovenous fistula puncture during the statistical period to the total number of arteriovenous fistula puncture procedures.”
“Number of correct physical examinations performed before arteriovenous fistula puncture/Total number of arteriovenous fistula puncture cases within the statistical period * 100%”

About these checks
Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.
Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-10-27.
Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-10-27.
Cited sources are checked by gpt-6-sol.
Statistics and arithmetic are recomputed, not judged.
Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
