Inclisiran SiRNA therapy for durable LDL-C reduction: a systematic review and meta-analysis highlighting a breakthrough in long-term cardiovascular risk management
Open accessAbstract
Background: Inclisiran is a novel small interfering RNA therapeutic developed to lower lipid levels by targeting hepatic production of proprotein convertase subtilisin/kexin type 9. Unlike monoclonal antibodies, Inclisiran silences messenger RNA expression, resulting in sustained reductions in low-density lipoprotein cholesterol with infrequent dosing. This study aimed to systematically evaluate the efficacy and clinical applicability of Inclisiran across randomized controlled trials. Methods: A systematic literature search was conducted in PubMed, Scopus, Embase, and Cochrane CENTRAL databases up to December 2024. This meta-analysis updates the global evidence base by incorporating 14 randomized trials (> 13,000 participants) identified through December 2024. Beyond earlier syntheses, it provides pooled analyses of multiple lipid parameters, phenotype-stratified subgroup and sensitivity analyses, and meta-regression for baseline LDL-C and statin use, thereby offering the most comprehensive and clinically differentiated evaluation of inclisiran to date. The primary outcome was the mean change in low-density lipoprotein cholesterol levels in patients treated with Inclisiran compared to placebo. Data was pooled using a random-effects model. Heterogeneity was assessed using the I² statistic, and publication bias was evaluated with funnel plot analysis. Results: Inclisiran treatment resulted in a significant mean reduction of 44.9% in low-density lipoprotein cholesterol levels compared to placebo (95% confidence interval: 39.54% to 50.25%; p < 0.001). While statistical heterogeneity was high (I² = 90.3%), visual assessment of the funnel plot suggested minimal risk of publication bias. Subgroup analyses supported consistent efficacy across patient populations, including those with statin intolerance or poor adherence to conventional therapies. Conclusions: Inclisiran provides a durable, twice-yearly injectable option for lipid management, offering substantial reductions in low-density lipoprotein cholesterol. Its unique RNA-based mechanism and long-acting profile make it particularly suitable for patients who struggle with adherence or have limited response to traditional lipid-lowering medications. These findings support Inclisiran’s role as a promising addition to current strategies for cardiovascular risk reduction.

TL;DR
This systematic review and meta-analysis of 14 randomized trials (>13,000 participants; 11 quantitatively pooled) found inclisiran reduced LDL-C by 44.9% versus placebo/control, with durable effects and safety comparable to control. It supports inclisiran as a twice-yearly option across diverse hypercholesterolemia populations, though cardiovascular outcome benefit remains unproven.
Key findings
- Pooled LDL-C reduction was 44.9% (95% CI 39.54%–50.25%; p < 0.001) across 11 RCTs with >13,000 participants; heterogeneity was high (I² = 90.3%), and sensitivity analysis excluding ORION-5 gave a pooled estimate of 46.22% (95% CI 41.02–51.42).
- Pooled PCSK9 reduction was 79.99% (95% CI 77.12%–82.86%; p < 0.001; I² = 50.8%); secondary lipid changes included non-HDL-C ↓38–50%, apoB ↓36–48%, lipoprotein(a) ↓15–20%, triglycerides ↓10–15%, and HDL-C ↑2–4%.
- Subgroup/durability findings: HeFH ORION-9 LDL-C ↓47.9% at Day 510; HoFH ORION-5 PCSK9 ↓60.6% but LDL-C change −1.68% (P = 0.90); ORION-8 showed 49.4% LDL-C reduction over median 3.7 years, with 78.4% achieving guideline targets; ORION-10/11 reductions exceeded 50%.
- Safety: pooled treatment-emergent adverse event risk ratio was 1.00 (95% CI 0.97–1.04; p = 0.80; I² = 0%); injection-site reactions occurred in 5.0%–7.5%, discontinuation due to adverse events was <2%, and anti-drug antibodies in ORION-1 were ~5.5% without attenuating LDL-C efficacy.
Methods
- Systematic search of PubMed, Scopus, Embase, and Cochrane CENTRAL through December 2024, following PRISMA 2020; the review was not prospectively registered in PROSPERO. Included RCTs in adults with hypercholesterolemia, familial hypercholesterolemia, or increased cardiovascular risk receiving inclisiran versus placebo or standard lipid-lowering therapy and reporting LDL-C; excluded non-randomized studies, animal/in vitro models, no control, insufficient data, and non-peer-reviewed articles. ORION-8 was included only qualitatively because of its open-label extension design.
- Fourteen RCTs (>13,000 participants) were included, with 11 RCTs used for quantitative synthesis. LDL-C was pooled using a DerSimonian–Laird random-effects model; heterogeneity was assessed with I²; subgroup and meta-regression analyses examined phenotype, baseline LDL-C, and statin use; sensitivity analyses excluded HoFH trials. Risk of bias used RoB 2; publication bias was assessed with funnel plot, Egger’s regression, and Begg’s rank-correlation tests. Safety used Mantel–Haenszel random-effects pooled risk ratios for treatment-emergent adverse events.
Limitations
- Stated: high heterogeneity (I² = 90.3%) from differences in populations, baseline LDL-C, comorbidities, follow-up duration, and concomitant lipid-lowering therapy; small trials had weak power and wide uncertainty intervals; possible selection bias remained despite funnel plot symmetry; only surrogate lipid outcomes were assessed, not direct long-term cardiovascular events; exclusion criteria may limit generalizability; the review was not prospectively registered in PROSPERO.
- Stated: no large-scale, fully published randomized trial has yet shown that inclisiran’s LDL-C reduction translates into MACE reduction; cardiovascular outcome benefits remain unproven; VICTORION-Outcomes is ongoing; formal cost-effectiveness analyses are warranted.
- Evident from the text: ORION-8 and other open-label extension data were not quantitatively pooled, limiting durability estimates; the meta-analysis includes post hoc pooled analyses and subanalyses, and high heterogeneity persisted after sensitivity analyses, so the pooled effect should be interpreted cautiously.

Inclisiran targets messenger RNA (mRNA) of proprotein convertase subtilisin/kexin type 9 (PCSK9) within hepatocytes to decrease PCSK9 synthesis at the transcriptional level.
“Through its application of RNA interference (RNAi) technology Inclisiran targets messenger RNA (mRNA) of proprotein convertase subtilisin/kexin type 9 (PCSK9) within hepatocytes to decrease PCSK9 synthesis at the transcriptional level [64, 65].”

The ‘PCSK9 as a promising therapeutic target for CVD’ section says inclisiran degrades PCSK9 mRNA in the liver and suppresses its translation. The ‘Targeting PCSK9 for cancer treatment’ section describes siRNA gene silencing as post-transcriptional, contrary to the claim that inclisiran decreases synthesis at the transcriptional level. Section 5 says inclisiran is a small interfering RNA taken up by liver cells; its guide strand directs cleavage and degradation of PCSK9 mRNA, preventing hepatic PCSK9 synthesis. Healthcare professionals’ perspectives on the use of PCSK9 inhibitors in cardiovascular disease: an in-depth qualitative study. Eur J Cardiovasc Nurs. 2024;23(8):919–24. source
The Introduction (p. 2) describes PCSK9 inhibitors as effective and says they require regular lifelong injections. The paper discusses barriers to prescribing, but the passages found do not say that injection frequency or high price restricted healthcare providers’ acceptance; its Theme 4 (p. 4) instead identifies guidance and organizational barriers. The Discussion reports that trial data demonstrated PCSK9 inhibitor effectiveness, and the Conclusions link high cost to prior authorization, high copays, and reduced patient access. The paper does not say that frequent injections or price restricted acceptance by healthcare providers; it examines whether patients received prescriptions their providers had already written. About these checks
Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.
Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-10-19.
Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-10-19.
Cited sources are checked by gpt-6-sol.
Statistics and arithmetic are recomputed, not judged.
Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
