Abstract

Gut-liver-axis (GLA) is a fundamental interaction between the gut and liver for maintaining human health. To clarify the physiological and pathological roles of GLA in the human body, a GLA microphysiological system (GLA-MPS) holds great potential. However, in current GLA-MPS, the importance of a physiologically relevant flow for gut and liver cells’ cultivation is not fully addressed. In addition, the integration of individual organ perfusion, circulation flow, and organ tissue functions in a single device has not been achieved. Here, we introduce a GLA-MPS by integrating two cell culture chambers with individually applied perfusion flows and a circulation channel with an on-chip pneumatic micropump under cell culture chambers via a porous membrane for interconnecting them. We analyzed the fluid shear stress (FSS) with computational fluid dynamics simulations and confirmed that the physiologically relevant FSS (i.e., 8 × 10 −3 and 1.2 × 10 −7 dyne cm −2 ) could be applied for the gut (Caco-2) and liver (HepG2) cells, respectively. Under physiologically relevant flow, the Caco-2 and HepG2 cells in the GLA-MPS maintained a cell survival rate of 95% and 92%, respectively; further, they enhanced the expression of functional proteins such as zonula occludens 1 (ZO-1) and albumin (ALB), respectively. Thus, the presented GLA-MPS can be adapted as an advanced in vitro model in a wide range of applications for disease modeling associated with inter-tissue interactions, such as fatty liver diseases.

TL;DR

The paper presents a four-layer gut-liver-axis microphysiological system (GLA-MPS) that applies separate perfusion flows to Caco-2 gut and HepG2 liver chambers while connecting them by on-chip micropump-driven circulation through a porous membrane. It reports physiologically relevant fluid shear stress, high cell viability, enhanced functional protein expression, and barrier function under optimized flow.

Key findings

  • CFD estimated apical gut FSS of 8 × 10⁻³ to 2 × 10⁻² dyne cm⁻² under 2–5 μL min⁻¹ perfusion, and basal/circulation FSS of 1 × 10⁻⁸ to 1.2 × 10⁻⁷ dyne cm⁻² through a 20-μm-thick porous membrane.
  • After 8 days with circulation flow, Caco-2 and HepG2 cells showed 95% and 92% viability, respectively.
  • ZO-1 expression in Caco-2 increased with gut perfusion at 5 μL min⁻¹ (FSS = 2 × 10⁻² dyne cm⁻²), while albumin expression in HepG2 increased when circulation flow increased from 0 to 61 nL min⁻¹.
  • The micropump produced 22–61 nL min⁻¹ circulation at 1 Hz and 50–150 kPa, giving a 1.5%–9% exchange ratio in 1 min versus approximately 0.25%–1% in other GLA-MPSs; the device required only 1.5 × 10⁴ cells per chamber versus 3 × 10⁵ for 24-well inserts.

Methods

  • The GLA-MPS was fabricated from four PDMS layers plus glass: a cell-culture layer with chambers (5.3 mm × 2 mm × 360 μm), a porous membrane (5-μm holes, 20-μm pitch and thickness), a circulation layer, and a control layer with an on-chip pneumatic micropump.
  • Caco-2 and HepG2 cells were cultured in separate chambers, with gut apical perfusion and closed-loop circulation; readouts included CFD with COMSOL, microbead flow-rate tracing, cell viability assay, immunocytochemistry for ZO-1 and albumin, and FITC-dextran permeability.
  • Statistical analysis used Tukey-Kramer and asymptotic Wilcoxon signed-rank tests; flow-rate measurements are reported as mean ± SD (n = 10), and permeability assay as n = 3.

Limitations

  • The paper states that the device could be improved by integrating organoid structures, by controlling molecular transport gradients across barrier tissues, and by adding TEER sensors with better accuracy and reproducibility; it also notes that the fluorescent dye used for barrier testing might influence cellular functions.
  • Evident from the text: the study used Caco-2 and HepG2 cell lines rather than patient-derived primary cells, and the authors note that primary cells could have patient-to-patient variation; microbiome and other tissue-cell applications are proposed as future work rather than demonstrated here.

Effective drugs or treatments for IBD and NAFLD are not available because of the lack of mechanistic insights.

“Although it is well known that IBD 7 and NAFLD 35 are strongly associated with the gut microbiome, effective drugs or treatments are not available in the market because of the lack of mechanistic insights.”

Patient-derived primary cells could have large patient-to-patient variations.

“Since patient-derived primary cells could have large patient-to-patient variations, it would be beneficial to obtain such standard datasets with the use of commonly used cell lines.”

About these checks

Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.

Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-11-06.

Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-11-06.

Cited sources are checked by gpt-6-sol.

Statistics and arithmetic are recomputed, not judged.

Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
