Effects of intranasal oxytocin on social anxiety in males with fragile X syndrome
Open accessAbstract
Summary Fragile X syndrome (FXS) is a rare inherited genetic disorder causing severe intellectual disability and autistic-like symptoms. Individuals with FXS, males in particular, often exhibit extreme eye gaze avoidance and hyperarousal when they encounter stressful social situations. We investigated whether oxytocin (OT), a hormone with prosocial and anxiolytic effects, could alleviate symptoms of social anxiety in this population. A randomized double-blind placebo-controlled single-dose trial was performed with intranasal administration of placebo, 24 IU OT and 48 IU OT. Measures of eye gaze frequency, heart rate, respiratory sinus arrhythmia (RSA), heart rate variability (HRV) and salivary cortisol were obtained during a structured social challenge conducted 50 min following OT administration. Ten low-functioning males with FXS (aged 13–28 years) traveled to Stanford for the initial visit: 8 completed the study. Eye gaze frequency improved significantly in response to the 24 IU OT dose and salivary cortisol levels decreased significantly in response to the 48 IU OT dose. There was no effect of OTon heart rate, RSA or HRV although individual plots of the heart rate data suggested that OT increased heart rate in some participants and decreased heart rate in others. These findings suggest that intranasal administration of OT may ameliorate some symptoms of social anxiety in patients with FXS. Further double-blind placebo-controlled studies of OT, conducted in combination with behavioral treatment programs, may be warranted.

TL;DR
In a randomized, double-blind, placebo-controlled single-dose trial, intranasal oxytocin improved eye gaze frequency at 24 IU and lowered salivary cortisol at 48 IU in eight low-functioning males with fragile X syndrome. Oxytocin did not affect heart rate, respiratory sinus arrhythmia, or heart rate variability.
Key findings
- Eye gaze frequency showed a significant main effect of oxytocin dose [F(2,10) = 4.82, p = 0.034, partial η² = .49], with post hoc improvement after 24 IU versus placebo (p = .042); the mean improvement was low, about 6 additional eye gaze occurrences during social interaction.
- Salivary cortisol showed a significant main effect of oxytocin dose [F(2,12) = 6.78, p = 0.011, partial η² = .53], with post hoc decrease after 48 IU versus placebo (p = 0.05).
- There was no effect of oxytocin on heart rate, RSA, or HRV; individual heart rate plots suggested some participants increased and others decreased heart rate.
- No adverse events were reported, though participant P7 developed Bell’s Palsy a few days after his first visit and his second visit was delayed by two months as a precaution.
Methods
- Randomized double-blind placebo-controlled single-dose crossover trial: each subject visited the lab three times, 7 days apart, receiving placebo, 24 IU intranasal Syntocinon, or 48 IU intranasal Syntocinon in random order; a 10-min videotaped social challenge was conducted 50 min after dosing.
- Participants were ten enrolled male adolescents/adults aged 13–28 years with confirmed fully methylated full mutation fragile X syndrome; eight completed/analyzed (mean age 21.3 years, SD = 5.1; Vineland-II Adaptive Behavior Composite range 31–58, mean = 43.0, SD = 12.2).
- Measures included eye gaze frequency, heart rate, RSA, HRV, and salivary cortisol before and 20 min after the social challenge; repeated measures ANOVA with age as covariate for cortisol/cardiac measures and age plus Vineland-II score for eye gaze, with Tukey’s LSD post hoc tests and p < 0.05.
Limitations
- Stated: the sample was very small (eight analyzed) because most males with FXS take psychoactive medications and parents were reluctant to discontinue them; six of eight were medication-free, one remained on a low-dose antidepressant, and one took an antiepileptic, though the two medicated participants did not appear to differ in response.
- Stated: only a single dose of oxytocin was administered, so longer administration was not tested.
- Stated: response variability and magnitude were concerns; it was unclear why 24 IU improved eye gaze but not 48 IU, while 48 IU lowered cortisol but not 24 IU, and the mean eye gaze improvement was small. Evident: the study had no typically developing or non-FXS comparison group.

About these checks
Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.
Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-10-09.
Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-10-09.
Statistics and arithmetic are recomputed, not judged.
Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
