Repeated Social Defeat, Neuroinflammation, and Behavior: Monocytes Carry the Signal

Open access

Abstract

Mounting evidence indicates that proinflammatory signaling in the brain affects mood, cognition, and behavior and is linked with the etiology of psychiatric disorders, including anxiety and depression. The purpose of this review is to focus on stress-induced bidirectional communication pathways between the central nervous system (CNS) and peripheral immune system that converge to promote a heightened neuroinflammatory environment. These communication pathways involve sympathetic outflow from the brain to the peripheral immune system that biases hematopoietic stem cells to differentiate into a glucocorticoid-resistant and primed myeloid lineage immune cell. In conjunction, microglia-dependent neuroinflammatory events promote myeloid cell trafficking to the brain that reinforces stress-related behavior, and is argued to play a role in stress-related psychiatric disorders. We will discuss evidence implicating a key role for endothelial cells that comprise the blood-brain barrier in propagating peripheral-to-central immune communication. We will also discuss novel neuron-to-glia communication pathways involving endogenous danger signals that have recently been argued to facilitate neuroinflammation under various conditions, including stress. These findings help elucidate the complex communication that occurs in response to stress and highlight novel therapeutic targets against the development of stress-related psychiatric disorders.

TL;DR

This review argues that repeated social defeat (RSD) drives sympathetic and HPA-axis outflow that biases hematopoiesis toward primed, glucocorticoid-resistant Ly6C^hi monocytes, which traffic to stress-responsive brain regions and reinforce neuroinflammation and anxiety-like behavior. It synthesizes rodent and human evidence for monocyte-dependent brain-immune communication and highlights endothelial cells, chemotactic axes, and danger signals as therapeutic targets.

Key findings

  • RSD increases circulating Ly6C^hi monocytes beginning after 3 cycles and robustly after 6 cycles; these cells are primed, glucocorticoid-resistant, and traffic to threat-appraisal regions including frontal cortex, hypothalamus, amygdala, and hippocampus.
  • Blocking monocyte trafficking via CCR2 or CX3CR1 knockout prevented monocyte recruitment to the brain and blocked anxiety-like behavior, while RSD still released Ly6C^hi monocytes into circulation.
  • Anxiety-like behavior and neuroinflammatory markers persist at least 8 days after RSD but are no longer apparent after 24 days; reexposure to a subthreshold stressor at 24 days reactivates monocyte trafficking and anxiety-like behavior via spleen-derived monocytes.
  • Human evidence includes chronically stressed caregivers showing diminished glucocorticoid-receptor-mediated transcription, heightened NF-κB-mediated transcription, and approximately twofold higher circulating C-reactive protein; about one-third of gene-expression differences in low-SES humans overlapped with RSD mice.

Methods

  • Narrative review; no new primary data are reported.
  • Synthesizes rodent RSD studies and human studies of chronic stress, including caregivers, low socioeconomic status, depressed suicide victims, and a longitudinal sample of 147 healthy young women interviewed every 6 months for 2.5 years.
  • Main analysis is a conceptual synthesis linking sympathetic/HPA outflow, myelopoiesis, monocyte trafficking, blood-brain barrier endothelium, microglial activation, behavior, and danger-signal pathways.

Limitations

  • Stated: mechanisms driving neuron-to-microglia communication are not completely understood; further studies are needed to determine whether MMP-mediated mechanisms regulate monocyte entry into brain parenchyma; more studies are needed before conclusions can be made about leukocyte recruitment to the CNS in chronically stressed or depressed humans.
  • Evident: as a review, it does not experimentally test the proposed causal pathway and relies heavily on rodent RSD findings, with human evidence largely observational or correlational.

About these checks

Registry, retraction, Cites and statistics results are computed, not judged, with one exception: a printed reference that no index lists is read once by a model before it counts as a citation error. Claims are extracted by a model and kept only when their quote appears verbatim in the text; each cited source is then checked, with a model’s help, for whether it says what the claim relies on. Those verdicts can be wrong — read the claim’s quote and the cited source before relying on one.

Claims read by deepseek-v4.1-flash under definition version 3 (extract-v3) on 2026-10-09.

Classified by jev-1.13-20260917 (gold-guideline-v0.2) on 2026-10-09.

Statistics and arithmetic are recomputed, not judged.

Confidence bands are shown only for fields calibrated on our annotated test set. None is calibrated yet, so no band is shown.
